A return to work certificate, also called a fitness for work or medical clearance note, confirms you are well enough to go back to your job. From Waterloo, you can have the full assessment by secure video or phone with a Canadian licensed provider. The assessment visit is $80, and the $60 certificate fee is charged only if you are cleared.
Book Return to Work Assessment$80 assessment visit · $60 certificate only if approved

TelePlus Care issues return to work certificates in Waterloo only after a full assessment with a Canadian licensed provider. The assessment visit is $80. The $60 certificate fee applies only if you are cleared to return; otherwise there is no certificate fee.
Clinical content is reviewed for accuracy and scope. TelePlus Care appointments go ahead only when a doctor agrees it is the right call, after a real assessment, and the doctor will explain the next step if virtual care is not the right fit.
Co-op terms and exam deadlines in Waterloo rarely wait for a clinic opening, and a placement employer may want medical clearance after time away. If you have been off with an illness or injury, a provider can assess your recovery and your duties by secure video or phone. Booking from home makes it easier to sort out before your next term or shift.
Employers often ask for this instead of a sick note. A sick note explains an absence, while the certificate confirms you can safely go back, names the date you return and can set out temporary restrictions, for example lighter work or reduced hours.

The certificate is never issued on request alone. A provider first needs to understand what happened, how you are recovering and what your job involves.

Pricing comes in two parts. The assessment visit is billed like any other TelePlus Care appointment, and the certificate fee applies only when the provider clears you.
Sometimes it is too early to go back, or the provider needs more information first, such as a specialist report, test results or an in-person exam. In that case you are not charged the $60 certificate fee, and the provider explains the next step. That may be more time off, a follow-up visit, or seeing a doctor in person.
Grand River Hospital and St. Mary's General serve the city and the surrounding region if the provider wants you seen in person. For emergencies, call 911 or head to the nearest emergency department, and Health811 (dial 811) gives free nurse advice when you are not sure where to go.

Bringing these to the visit usually means the provider can decide the same day.
