A return to work certificate, also called a fitness for work or medical clearance note, confirms you are well enough to go back to your job. From Surrey, you can have the full assessment by secure video or phone with a Canadian licensed provider. The assessment visit is $80, and the $60 certificate fee is charged only if you are cleared.
Book Return to Work Assessment$80 assessment visit · $60 certificate only if approved

In Surrey, a TelePlus Care return to work certificate is issued only after a full assessment visit with a Canadian licensed provider. The assessment visit is $80. If the provider clears you to return, the certificate is $60. If not, there is no certificate fee.
Clinical content is reviewed for accuracy and scope. TelePlus Care appointments go ahead only when a doctor agrees it is the right call, after a real assessment, and the doctor will explain the next step if virtual care is not the right fit.
Trades and logistics jobs in Surrey often call for a documented return-to-work date before you are back on the job. After an illness or injury, that usually means a medical clearance from a provider. The full assessment is done by secure video or phone, so you can get it started from home.
Employers often ask for this instead of a sick note. A sick note explains an absence, while the certificate confirms you can safely go back, names the date you return and can set out temporary restrictions, for example lighter work or reduced hours.

The certificate is never issued on request alone. A provider first needs to understand what happened, how you are recovering and what your job involves.

You pay for the assessment the same way as any TelePlus Care visit. The certificate itself is a separate charge, and it applies only if you are approved to return.
The provider may decide you need more recovery time, or may want more information first, such as test results, a specialist report or an in-person exam. If so, the $60 certificate fee is not charged, and you are told what comes next: more time off, a follow-up visit or an in-person appointment.
Surrey Memorial and Peace Arch hospitals serve Guildford, Newton, Cloverdale and the south of the city if the provider wants you examined in person. 8-1-1 HealthLink BC offers free nurse advice when you are unsure where to go.

Bringing these to the visit usually means the provider can decide the same day.
